Where to obtain your immunization records

  1. An official copy of your immunization record from your private physician or a health clinic where your received your childhood vaccinations   
    -OR-
  2. A copy of your high school or previous college’s immunization record                               
    -OR-
  3. A copy of blood titer levels which can be drawn by your private physician to see if you have immunity to measles, mumps, rubella and Hepatitis B (if applicable). You must submit the lab titer from the lab such as LabCorp or Quest.

  4. The Docket app
    • Docket is a new app from the NJ Department of Health that makes it easier for you and your family to securely access your vaccination record.
    • The Docket app is now available in the App store or on Google Play in English or Spanish depending on Smartphone settings.

All immunization records are to be uploaded to the Healthy Bronc portal.
