All students must have the emergency form completed and turned  into the performance management office at least two weeks before the  first off-campus performance. Any students who have not provided this form cannot participate in off-campus performances until it is submitted.

All students who choose not to use the provided transportation to an off-campus performance must complete a travel form in the Performance Management office at least 24 hrs before the departure date.
