Course Book Adoption Form

You may download the form (PDF) and return it to the Bookstore via interoffice mail or via fax.  You can complete the PDF form (see areas in red) and easily print it. If you have Adobe Acrobal Professional installed on your machine, you can save the PDF once you have filled it out and simply email the form to bmichals@rider.edu.
